Фінансове забезпечення діяльності закладів охорони здоров’я в умовах їх реформування

Abstract

Кваліфікаційна робота присвячена дослідженню та аналізу фінансового забезпечення закладів охорони здоров'я у контексті сучасних реформувань у галузі охорони здоров'я. Система фінансування медичних установ вважається критичною для забезпечення якісних та доступних медичних послуг населенню. В роботі розглядаються ключові аспекти фінансового забезпечення, такі як джерела фінансування, включаючи державний та приватний сектори, механізми розподілу фінансових ресурсів між різними ланками системи охорони здоров'я, ефективність використання бюджетних коштів, а також питання фінансової стійкості закладів охорони здоров'я. У роботі використовуються різноманітні методи дослідження, зокрема аналіз фінансових звітів, порівняльний аналіз різних моделей фінансування, а також економічний аналіз ефективності витрат. Дослідження зосереджується на пошуку оптимальних стратегій фінансування, що сприятимуть підвищенню якості медичних послуг та забезпечать фінансову стійкість закладів охорони здоров'я в умовах постійних змін. Результати цього дослідження можуть бути корисними для розробки та впровадження політик управління фінансовими ресурсами в галузі охорони здоров'я, спрямованих на покращення ефективності та доступності медичних послуг для населення. Такий підхід є важливим у контексті постійного розвитку та модернізації системи охорони здоров'я з метою задоволення потреб суспільства у якісних медичних послугах. З метою здійснення дослідження для аналізу обрано 5 медичних закладів різного спрямування з огляду на медичні послуги: КНП "Київська міська клінічна лікарня швидкої медичної допомоги" КМДА, КНП "Дитяча клінічна лікарня № 5 Святошинського району міста Києва " КМДА, КНП "Київська міська лікарня № 5" КМДА, КНП "Міська дитяча клінічна інфекційна лікарня" КМДА, КНП "Міський клінічний ендокринологічний центр" КМДА. Аналіз свідчить, що існуючий фінансовий стан медичної галузі потребує удосконалення та оптимізації. Необхідність реформ в системі охорони здоров'я підтверджується проблемами, виявленими в процесі здійснення дослідження. Визначено необхідність забезпечення прозорості та відкритості в управлінні фінансами медичних установ, а також встановлення механізмів відповідальності за недосягнення цілей ефективності, необхідність визначення пріоритетів у використанні фінансових ресурсів з метою підвищення ефективності медичної галузі та поліпшення результатів у наданні медичних послуг. Державні лікарні в Україні переважно фінансуються за рахунок державних коштів. Це свідчить про те, що лікарні не є привабливими для інвесторів і залежать від підтримки держави. Лікарні мають достатньо власних коштів для забезпечення своєї діяльності. Для підвищення ефективності діяльності державних лікарень необхідно зробити лікарні більш привабливими для інвесторів. Запропоновано впровадження ефективних систем управління, вдосконалення надання медичних послуг та підвищення якості роботи лікарів. Тaкoж пpaцює пpoгpaмa мeдичниx гapaнтiй, вiдпoвiднo дo якoï виплaтy oтpимyє 6eзпocepeднiй нaдaвaч пocлyги. Зaпpoпoнoвaнo зaлyчeння cтpaxoвиx кoмпaнiй. Впpoвaджeння нaдaниx пpoпoзицiй дoзвoлить cyттєвo пiдвищити piвeнь oxopoни здopoв'я в кpaïнi, змeншити нaвaнтaжeння нa 6юджeт, пiдвищити дocтyпнicть oтpимaння мeдичниx пocлyг тa ïx якicть. Introduction. The qualification work is devoted to the research and analysis of the financial support of health care institutions in the context of modern reforms in the field of health care. The system of financing medical institutions is considered critical for providing quality and affordable medical services to the population. The work considers key aspects of financial support, such as sources of funding, including the public and private sectors, mechanisms for the distribution of financial resources between different parts of the health care system, the effectiveness of the use of budget funds, as well as the issue of financial sustainability of health care institutions in a changing environment . The work uses a variety of research methods, including analysis of financial statements, comparative analysis of various financing models, and economic analysis of cost effectiveness. The research focuses on the search for optimal financing strategies that will contribute to the improvement of the quality of medical services and ensure the financial stability of health care institutions in conditions of constant change. The results of this study can be useful for the development and implementation of policies for the management of financial resources in the field of health care, aimed at improving the efficiency and accessibility of medical services for the population. This approach is important in the context of continuous development and modernization of the health care system in order to meet the needs of society in quality medical services. Purpose. For the purpose of carrying out the research, 5 medical institutions of different orientations were chosen for analysis in terms of medical services: KNP "Kyiv City Clinical Hospital of Emergency Medical Aid" KMDA, KNP "Children's Clinical Hospital No. 5" KMDA, KNP "City Children's Clinical Infectious Hospital" KMDA, KNP "City Clinical Endocrinology Center" KMDA. Results. The analysis shows that the existing financial condition of the medical industry needs improvement and optimization. The need for reforms in the health care system is confirmed by the problems identified during the analysis. The need to ensure transparency and openness in managing the finances of medical institutions, as well as establishing accountability mechanisms for failure to achieve performance goals. The allocation of priorities in the use of financial resources in order to increase the efficiency of the medical industry and improve the results in the provision of medical services. State hospitals in Ukraine are mainly financed by state funds. This shows that hospitals are not attractive for investors and depend on state support. Hospitals have enough of their own funds to support their activities. To improve the efficiency of public hospitals, it is necessary to make hospitals more attractive to investors. This can be done, for example, by implementing effective management systems, improving the provision of medical services and improving the quality of work of doctors. Conclusions. The previous study showed that the main beneficiaries of the financing of the oxopons of the region are the elderly and non-governmental organizations, patients, youth funds, and children's aid. It also applies to medical guarantee programs, according to which payments are received by the 6th service provider. Proposed inclusion of three companies. The implementation of the proposal will allow us to significantly increase the level of access to health care in the country, reduce the burden on the budget, and increase the accessibility and quality of medical services.
