Особливості фінансування сфери охорони здоров'я: досвід Сінгапуру та можливості використання в національній практиці

Abstract

Магістерська робота присвячена комплексному аналізу особливостей фінансування сфери охорони здоров’я Сінгапуру та визначенню можливостей застосування успішних елементів цієї моделі в національній практиці України. Актуальність дослідження зумовлена необхідністю підвищення ефективності фінансових механізмів української системи охорони здоров’я в умовах структурних реформ, обмеженості бюджетних ресурсів та зростаючих вимог до якості медичних послуг. У роботі висвітлено еволюцію сінгапурської моделі фінансування, її стратегічні цілі та інституціональні засади, що забезпечили державі одну з найефективніших медичних систем у світі. Особливу увагу приділено аналізу ключових фінансових інструментів Сінгапуру – програмам Medisave, MediShield Life та MediFund. У роботі докладно охарактеризовано механізми їх функціонування, логіку взаємодії між громадянами, страховими фондами та державою, а також вплив цих інструментів на стабільність системи та рівень доступності медичної допомоги. Розглянуто принципи співфінансування, високий рівень персональної відповідальності громадян за власні медичні витрати, роль держави як регулятора та гаранта соціального захисту вразливих груп населення. За допомогою порівняльного аналізу в роботі узагальнено основні відмінності між українською та сінгапурською системами фінансування охорони здоров’я, включно зі структурою джерел фінансування, механізмами розподілу ресурсів, ступенем автономії медичних закладів та інструментами страхового захисту. Окремо розглянуто питання ефективності витрачання коштів, рівня прозорості, мотиваційних механізмів для медичних працівників та пацієнтів, а також фактори, що визначають суспільну довіру до медичної системи. На основі виявлених переваг сінгапурського досвіду сформульовано низку практичних рекомендацій для України. Серед них – удосконалення системи державного медичного страхування, розвиток програм довгострокових медичних накопичень, запровадження гнучких механізмів співоплати, підвищення адресності державної фінансової допомоги, а також посилення автономії та конкурентоспроможності медичних закладів. Акцент зроблено на необхідності адаптації зарубіжних практик з урахуванням соціально-економічних умов України, її інституційної структури та рівня фінансової стабільності населення. Отримані результати можуть бути використані органами державної влади, закладами охорони здоров’я, науковими установами та експертними організаціями у процесі розроблення та впровадження політики модернізації фінансування охорони здоров’я. Дослідження також має практичне значення для подальшого вдосконалення реформи медичної системи та підвищення ефективності управління галуззю. The master's thesis is devoted to a comprehensive analysis of the features of financing the healthcare sector in Singapore and determining the possibilities of applying successful elements of this model in the national practice of Ukraine. The relevance of the study is due to the need to increase the efficiency of financial mechanisms of the Ukrainian healthcare system in the context of structural reforms, limited budget resources and growing requirements for the quality of medical services. The work highlights the evolution of the Singaporean financing model, its strategic goals and institutional principles, which have provided the state with one of the most effective medical systems in the world. Special attention is paid to the analysis of key financial instruments of Singapore - the Medisave, MediShield Life and MediFund programs. The paper describes in detail the mechanisms of their functioning, the logic of interaction between citizens, insurance funds and the state, as well as the impact of these instruments on the stability of the system and the level of accessibility of medical care. The principles of co-financing, a high level of personal responsibility of citizens for their own medical expenses, the role of the state as a regulator and guarantor of social protection of vulnerable groups of the population are considered. Using a comparative analysis, the paper summarizes the main differences between the Ukrainian and Singaporean healthcare financing systems, including the structure of funding sources, resource allocation mechanisms, the degree of autonomy of medical institutions and insurance protection instruments. The issues of the efficiency of spending funds, the level of transparency, motivational mechanisms for medical workers and patients, as well as factors that determine public trust in the medical system are separately considered. Based on the identified advantages of the Singaporean experience, a number of practical recommendations for Ukraine are formulated. Among them are the improvement of the state health insurance system, the development of long-term medical savings programs, the introduction of flexible co-payment mechanisms, the increase in the targeting of state financial assistance, as well as the strengthening of the autonomy and competitiveness of medical institutions. The emphasis is on the need to adapt foreign practices taking into account the socio-economic conditions of Ukraine, its institutional structure and the level of financial stability of the population. The results obtained can be used by state authorities, health care institutions, scientific institutions and expert organizations in the process of developing and implementing a policy for the modernization of health care financing. The study also has practical significance for further improving the reform of the health care system and increasing the efficiency of industry management.
